Organizations must apply for certification in the category that accurately reflects their operational scope and service delivery model. Misrepresentation of category or services constitutes grounds for application denial or certification revocation.
Organizations operating multiple service lines or facility types must obtain separate certification for each applicable category. Each certification is evaluated independently and requires distinct documentation, certification fees, and ongoing monitoring.
Requirements listed for each category represent core standards applicable to all applicants. Additional documentation or verification may be required based on specific operational characteristics, geographic jurisdiction, or credentialing requirements applicable to the organization.
